APPLICATION FOR FINANCIAL ASSISTANCE IN 2026
All applications must be filled out and emailed to [email protected] in order for the approval process to begin, and a determination be made regarding your application.

Complete all sections of the application below.

Proof of income will be required before release of funds to approved applicants (check stubs, direct deposit, etc.).

Every applicant will be subject to providing proof of current medical status, that will determine his or her financial assistance approval (from a doctor).

Our grants are given out on a first come, first serve basis, and will begin on April 15th, 2026 and ending on December 12th, 2026.
